Questionnaire form completed by an unnamed Lieutenant Colonel as part of the IDFFT fact finding process. The Lieutenant Colonel had no contact with Prisoners of War and civilian detainees.  The Lieutenant Colonel visited Abu Ghraib and was privy to a discussion with his superior officer in relation to there being an investigation into abuse at Abu Ghraib.
